We are only able to accept patients who live within our Practice area. This is represented by a semi-circle of 4-5 mile radius from Douglas town centre. To the North it extends as far as Baldrine, West Crosby and to the South, Santon.
Registration application forms are available at reception and require to be fully completed by a patient before we can commence the registration process, together with a registration medical information form and standard ethnicity questionnaire, to ensure that we can meet any needs determined.
Once these have been returned to reception, our receptionists will invite you to make an appointment for a registration medical with one of the nurses. This 20 minute appointment is very important as it takes the form of a mini medical and allows us to gain valuable information from you regarding your health and medication as it can take up to 6 weeks before we receive your GP notes from your previous Practice.
